Required Forms for Fleer Student
All Fleer Center
students admitted are required to submit, as part of the application process, documentation of the immunization requirements based on the student’s age. Health Services has developed a Guideline for the Fleer Center Immunization Form to help future students complete necessary documentation.
Voluntary Health Care Plan
Fleer Center students at Salem College may purchase health insurance by enrolling in the voluntary plan. Fleer students do not do the waiver process.
